OCR Profile Account Transfer Form

This form is used when the management of an OCR profile needs to be transferred to a new or existing user in the Accelerate platform. The form should be completed, signed and returned via e-mail to OCR.support@fia-tech.com by either the existing "original" user/profile manager, or the "new" user taking over the profile management.  Physically signed or DocuSign electronic signatures are fine.  But signatures done by obviously typing with cursive font are not acceptable. 

OCR Joint User Management Form

This form is used when a profile manager for one or more OCR profiles needs to share profile management access with one or more other users. The form should be completed, signed and returned via e-mail to OCR.support@fia-tech.com for each joint user by the existing "original" profile manager. 

OCR Profile Selective Transfer Form

This form is used when a user would like to transfer management of some of their profiles to a different user but not all.  The form should be completed, signed and returned via e-mail to OCR.support@fia-tech.com by the existing "original" user/profile manager.
